A RESOLUTION AUTHORIZING THE CITY OF PUEBLO
TO ENTER INTO AN AGREEMENT WITH FHP /TAKECARE
AND SOUTHERN COLORADO HEALTH PLAN FOR FIREFIGHTER
HEALTH INSURANCE FOR THE CALENDAR YEAR 1995
WHEREAS, the Firefighters Union (IAFF Local #3) has voted
to recommend that the City contract with FHP /TakeCare and
Southern Colorado Health Plan for 1995, and
WHEREAS, they have received favorable rates from
FHP /TakeCare and Southern Colorado Health Plan; now therefore
BE IT RESOLVED BY THE CITY COUNCIL OF PUEBLO, COLORADO,
that:
SECTION 1.
The City Council authorize the City to enter into a
contract with FHP / Take Care/ Southern Colorado Health Plan for
Firefighter health insurance for the Calendar year 1995.
SECTION 2.
This resolution shall become effective immediately.

Dear City Council Members:
The Pueblo Firefighters have elected to contract with FHP/TakeCare as the exclusive
provider of medical benefits for the City of Pueblo Firefighters and their dependents, and
retirees. This decision was voted on and accepted by the membership. For the purpose of
this contract, FHP/TakeCare will utilize their unique relationship with Southern Colorado
Health Plan. The following rates will be effective for this new plan January 1, 1995.

9% rate cap for the plan year January 1, 1996.
