ORDINANCE NO. 11229
AN ORDINANCE APPROVING A LEASE AGREEMENT
BETWEEN THE CITY OF PUEBLO, COLORADO MUNICIPAL
CORPORATION, AND CROSSOVER HEALTH MEDICAL
GROUP, APC, A CALIFORNIA PROFESSIONAL
CORPORATION, RELATED TO THE OPERATION OF AN
EMPLOYEE HEALTH CLINIC, AND AUTHORIZING THE MAYOR
TO EXECUTE SAME
WHEREAS, the City desires to approve the Lease Agreement with Crossover Health
Medical Group, APC to manage a health clinic for employees in accordance with previously
approved agreements between the parties; NOW, THEREFORE,
BE IT ORDAINED BY THE CITY COUNCIL OF PUEBLO, that:
SECTION 1.
The Lease Agreement ("Agreement") between the City of Pueblo, a Colorado municipal
corporation ("City"), and Crossover Health Medical Group, APC, a California professional
corporation, for the operation of an employee health clinic, a copy of which is attached hereto,
having been approved as to form by the City Attorney, is hereby approved.
SECTION 2.
The Mayor is hereby authorized to execute said Agreement for and on behalf of the City,
and the City Clerk is directed to affix the seal of the City thereto and attest same.
SECTION 3.
The officers and staff of the City are authorized and directed to perform any and all acts
consistent with this Ordinance and the attached Agreement to implement the policies and
procedures described herein.
SECTION 4.
This Ordinance shall become effective on the date of final action by the Mayor and City
Council.

PROFESSIONAL CORPORATION, RELATED TO THE OPERATION OF
AN EMPLOYEE HEALTH CLINIC, AND AUTHORIZING THE MAYOR TO

SUMMARY:
The attached Ordinance approves a Lease Agreement (the "Agreement") between the
City of Pueblo, Colorado, and Crossover Health Medical Group, APC, a California
Professional Corporation, for the operation of an employee health clinic in the Dittmer
Building. The Agreement provides for a three-year initial term commencing June 1,
2026, and expiring on the third anniversary of the Statement of Work Opening Date.
Per the prior agreements between the City and Crossover Health, the City is
responsible for all costs associated with the operation of the employee health clinic.
PREVIOUS COUNCIL ACTION:
On November 10, 2025, City Council approved Resolution No. 16171 which accepted
and approved a Professional Services Agreement ("PSK) and Statement of Work
("SOW') between the City of Pueblo and Crossover Health related to the operation of
the employee health clinic with an annual cost of $938,386. On April 13, 2026, City
Council approved Ordinance No. 11144 which created Project No. C12613 and
budgeted and appropriated funds thereto in the amount of $60,000 for the purpose of
funding necessary renovations to the Dittmer Building for the employee health clinic. On
May 11, 2026, City Council approved Ordinance No. 11173 which budgeted and
appropriated funds in the amount of $520,000 for the implementation and
administration of the employee health clinic for fiscal year 2026.

BACKGROUND:
The City has entered into an agreement with Crossover Health to operate a medical
clinic to benefit City employees enrolled in the City's healthcare insurance program and
their dependents. That clinic will be operated on the first floor of the City -owned
"Dittmer Building" located at 650 Dittmer Avenue under an agreement approved by City
Council on May 11, 2026. The lease agreement presented for approval under this
ordinance will provide Crossover Health with the space required to operate that clinic
and provide medical services at that location.
FINANCIAL IMPLICATIONS:
Per the prior agreements between the City and Crossover Health, the City is
responsible for all facility -related costs associated with the operation of the employee
health clinic.
BOARD/COMMISSION RECOMMENDATION:
Not applicable to this Ordinance.
STAKEHOLDER PROCESS:
Not applicable to this Ordinance.
ALTERNATIVES:
Failure to approve the Ordinance will preclude the City from executing the Lease
agreement, resulting in an inability to open the health clinic and satisfy related
obligations under other agreements.
RECOMMENDATION:
Approval of the Ordinance.
