HomeMy WebLinkAbout10463ORDINANCE NO. 10463 AN ORDINANCE APPROVING AND AUTHORIZING THE MAYOR TO EXECUTE AN ASSIGNMENT, ASSUMPTION AND AMENDMENT TO THE NURSE NAVIGATION AGREEMENT BETWEEN PUEBLO, A COLORADO MUNICIPAL CORPORATION, ACCESS2CARE, LLC, AND INNOVATIVE PRACTICES, LLC, AND SAME BE IT ORDAINED BY THE CITY COUNCIL OF PUEBLO, that: SECTION 1. The ASSIGNMENT, ASSUMPTION AND AMENDMENT TO NURSE NAVIGATION AGREEMENT (“Agreement”) between Pueblo, a municipal corporation, Access2Care, LLC, and Innovative Practices, LLC, attached hereto, having been approved as to form by the City Attorney, is hereby approved. SECTION 2. The Mayor is hereby authorized to execute said Agreement for and on behalf of the City and the City Clerk is authorized to affix the seal of the City thereto and attest the same. SECTION 3. The officers and staff of the City are authorized to perform any and all acts consistent with this Ordinance and the attached Agreement to implement the policies and procedures described therein. SECTION 4. This Ordinance shall become effective on the date of final action by the Mayor and City Council. Gradisar VIA: Marisa Stoller, City Clerk FROM: Barb Huber, Fire Chief SUBJECT: AN ORDINANCE APPROVING AND AUTHORIZING THE MAYOR TO EXECUTE AN ASSIGNMENT, ASSUMPTION AND AMENDMENT TO THE NURSE NAVIGATION AGREEMENT BETWEEN PUEBLO, A COLORADO MUNICIPAL CORPORATION, ACCESS2CARE, LLC, AND INNOVATIVE PRACTICES, LLC, AND SAME SUMMARY: This Ordinance approves a nurse navigation agreement between Access2Care LLC and the City of Pueblo and establishes Proj. PS2203 to manage grant funds related to such agreement. PREVIOUS COUNCIL ACTION: None. BACKGROUND: This is a 2-year pilot program grant that is through the Colorado Division of Homeland Security. It will help connect the community to nurse navigation services rather than utilizing 9-1-1 and emergency rooms. Willing callers will be sent to a nurse’s line after a thorough list of questions have identified an appropriate need. The long-term effect could be an improved use of connection to and use of medical resources as well as a reduction in 9-1-1 calls. FINANCIAL IMPLICATIONS: There is no cost to the city. Staff time will be required for project management, as well as grant monitoring and reporting. BOARD/COMMISSION RECOMMENDATION: Not applicable. STAKEHOLDER PROCESS: Not applicable.
