A RESOLUTION APPROVING ADDENDUM NO. 4 RELATING TO
A THREE MONTH EXTENSION OF THE AGREEMENT
BETWEEN THE CITY OF PUEBLO AND AMERICAN MEDICAL
RESPONSE OF COLORADO, INC. CONCERNING EMERGENCY
MEDICAL SERVICES
BE IT RESOLVED BY THE CITY COUNCIL OF PUEBLO, that:
SECTION 1.
The Addendum No. 4 to the Agreement between the City of Pueblo and American Medical
Response of Colorado, Inc., a true copy of which is attached hereto, having been approved as to
form by the City Attorney, is hereby approved.
SECTION 2.
The President of City Council is hereby authorized and directed to execute the Addendum
No. 4 on behalf of the City, and the City Clerk is directed to affix the seal of the City thereto and
attest same.
SECTION 3.
The officers of the City are directed and authorized to perform any and all acts consistent
the intent of this Resolution and the attached Addendum No. 4 to effectuate the policies and
procedures described therein.
SECTION 4.
This Resolution shall become effective immediately upon final passage.

EMERGENCY MEDICAL SERVICES
SUMMARY:
The attached resolution authorizes the City to extend the emergency medical services agreement
with American Medical Response of Colorado, Inc. (AMR) for an additional three month term from
October 31, 2017 through January 31, 2018.
PREVIOUS COUNCIL ACTION:
Approval of Addendum No. 1, Addendum No. 2, and Addendum No. 3.
BACKGROUND:
This resolution is for a three month extension of an existing agreement between the City of Pueblo
and American Medical Response of Colorado, Inc (AMR) on the same terms stated in the
Agreement. The City though its Fire Department, provides emergency patient care services to our
community without charge. When a patient is in need of transport to a hospital their care is
transferred to AMR and AMR is allowed to bill for their services. In certain cases, Fire Department
members assist AMR in the transport of patients to the hospital and this service is reimbursable.
This contract addresses the rate of reimbursement that is due to the City from AMR. The contract
also addresses response time criteria.

None.

ALTERNATIVES:

The alternative is to take action and allow the contract to expire. AMR would still maintain
their licensing to transport patients but would no longer be required to reimburse the City. This
would have a negative impact on the City budget of $8,020.00 per month.
RECOMMENDATION:
Approval of the Resolution.

ADDENDUM NO. 4.
This Addendum No. 4 is entered into this 23rd day of October, 2017 by and between Pueblo, a
municipal corporation ("City"), and American Medical Response of Colorado, Inc. ("AMR").
WHEREAS, City and AMR entered into an Agreement Concerning Emergency Medical Services
dated March 26, 2013 ("Agreement");
WHEREAS, City and AMR entered into Addendum No. 1 extending the term of the Agreement
for a one month period through May 31, 2017, Addendum No. 2 extending the term of the Agreement for
a two month period through July 31, 2017; and Addendum No. 3 extending the term of the Agreement for
a three month period through October 31, 2017.
WHEREAS, the parties wish to extend the term of the Agreement for an additional three-month
period.
NOW THEREFORE, in consideration of the foregoing and mutual covenants herein contained,
City and AMR agree as follows:
1. Section 6.1 of the Agreement is amended to read as follows:
6.1 The term of this Agreement shall be from May 1, 2013 through January 31,
2018.
3. Each person signing this Addendum No. 4 on behalf of a party represents and warrants that
he or she has the requisite power and authority to enter into, execute, and deliver this Addendum No. 4 on
behalf of such party and that this Addendum No. 4 is a valid and legally binding obligation of such party
enforceable against it in accordance with its terms.
4. The Agreement, as amended by this Addendum No. 4, shall remain in full force and effect.
IN WITNESS WHEREOF, AMR and City have executed this Addendum No. 4 as of the date first
written above.
