A RESOLUTION AUTHORIZING THE ISSUANCE OF A
REVOCABLE PERMIT TO APEX COMPANIES LLC FOR THE
INSTALLATION OF SOIL AND GROUNDWATER REMEDIATION
INFRASTRUCTUREWITHIN THE COURT STREET RIGHT-OF-
WAY BETWEEN 14TH STREET AND 15TH STREET
BE IT RESOLVED BY THE CITY COUNCIL OF PUEBLO, that:
SECTION 1.
The attached Revocable Permit is authorized to be issued to Apex Companies, LLC for
the installation of soil and groundwater remediation infrastructure within the Court Street right-of-
thth
way between 14 Street and 15 Street.
SECTION 2.
The use of the public right-of-way granted hereby is subject to all limitations, terms and
conditions set forth in the attached Revocable Permit.
SECTION 3.
The officers and staff of the City are directed and authorized to perform any and all acts
consistent with the intent of this Resolution to effectuate the policies and procedures described
herein.
SECTION 4.
This Resolution shall become effective immediately upon passage and approval.

SUMMARY:
Attached is an application for a revocable permit submitted by Apex Companies, LLC to install
soil and groundwater remediation infrastructure (wells and subsurface piping) within the Court
Street right-of-way between 14th Street and 15th Street.
PREVIOUS COUNCIL ACTION:
None.
BACKGROUND:
Apex Companies, LLC and its subcontractor, GDI Drilling, are requesting permission to install 5
remediation wells within the Court Street right-of-way between 14th Street and 15th Street. The
purpose of the remediation wells is to clean-up petroleum hydrocarbon-impacted soil and
groundwater, associated with a gasoline release being regulated and managed by the Division of
Oil and Public Safety (OPS) (Event lD 6531). Well installation is anticipated to take approximately
five days. Appropriate traffic control will be utilized per a subcontracted traffic control plan.
Concrete and/or asphalt coring, approximately 24-inches in diameter, will be used to access the
boring locations. Potholing will be performed to clear boring locations of utilities to 6-feet below
grade before drilling. Apex and GDI will then use hallow-stem augur technology to wells to
approximately 15 feet below
grade, per location.
FINANCIAL IMPLICATIONS:
None.
BOARD/COMMISSION RECOMMENDATION:
The Revocable Permit Review Committee recommends approval.
STAKEHOLDER PROCESS:
None.
ALTERNATIVES:
Denying the Resolution will prevent remediation of impacted soil and groundwater.

Dear Mr. Barela:
On behalf of the Division of Oil and Public Safety(OPS)of the Colorado Department of Labor and Employment,
Apex Companies, LLC(Apex)is requesting a revocable permit for remediation well installation and subsurface
trenching within the City of Pueblo public right-of-way(ROW), at the above referenced site (Figure 1). Permit
application fees will be submitted to the City of Pueblo upon submittal of this application.
Apex and its subcontractor, GDI Drilling, will install 5 remediation wells within the public ROW between
February 27 and March 3, 2017. The monitoring wells will be located within the Court Street right-of-way
between 14th Street and 15th Street, as shown in Figure 1.The purpose of the remediation wells is to clean-up
petroleum hydrocarbon-impacted soil and groundwater, associated with a gasoline release being regulated
and managed by the OPS (Event ID 6531). Well installation is anticipated to take approximately five days.
Appropriate traffic control will be utilized per a subcontracted traffic control plan (included with this
submittal).Concrete and/or asphalt coring, approximately 24-inches in diameter, will be used to access the
boring locations. Potholing will be performed to clear boring locations of utilities to 6-feet below grade before
drilling. Apex and GDI will then use hallow-stem augur technology to wells to approximately 15 feet below
grade, per location.
Thereafter,Apex and its subcontractor will perform subsurface trenching from an existing remediation shed to
the newly installed wells. Subsurface trenches will be approximately 12-inches in width by 24-inches deep
(Figure 2)and concrete and/or asphalt cutting will be used to access trenching locations. Remediation system
conduit (1-inch HDPE) will be laid within trenches, and trenches will be backfilled as presented in Figure 2.
Figure 3 presents the final well design and system connection.All surfaces will be completed to match existing.
